Breast Health ALIGN partners with hospitals and health systems to design community-integrated navigation strategies that improve access, strengthen equity outcomes, and support patients beyond clinic walls.
There are many factors that can delay breast health follow-up.
Transportation. Language access. Insurance instability. Fear. Trust. Coordination.
These challenges often exist in patients’ everyday lives—not just within healthcare settings. When they are not fully addressed, patients may experience delays or disruptions between screenings, diagnosis, and treatment
The result:
Hospitals are deeply committed to improving access and patient outcomes.
Strengthening how navigation connects clinical care with the real-world factors that influence follow-up can further support continuity across the care journey.
Breast Health ALIGN provides strategic consulting and program design services to help hospitals strengthen breast health navigation and improve equitable access to care.
The ALIGN™ Framework is a structured, experience-informed approach to designing and implementing community-integrated navigation models that improve access, strengthen patient engagement, and advance equity across the care continuum.
The ALIGN™ Framework is delivered through five integrated service areas:
Evaluate where systems break down—and why patients are lost.
A comprehensive assessment of navigation programs, access pathways, and system interactions to identify equity gaps, workflow barriers, and missed opportunities across clinical and community settings.
Build models that reflect how access actually happens.
Design community-integrated navigation frameworks that connect healthcare systems with trusted community-based organizations, extending navigation beyond clinical settings and into the environments where patients live.
Address the structural drivers of inequitable access.
Strategic advisory services to identify and address barriers such as transportation, language access, insurance instability, and trust—while aligning organizational priorities with patient needs and lived experiences.
Operationalize new approaches with clarity and confidence.
Structured support to translate strategy into practice, including implementation planning, workflow alignment, and early-stage refinement to ensure designed navigation models function effectively across settings.
Strengthen how patients move through care.
Strategic guidance to improve screening uptake, diagnostic follow-up, and care coordination—particularly for populations most at risk of being lost along the care continuum.
Breast Health ALIGN offers structured, phased engagement options designed to support health systems at different stages of readiness.
These offerings provide a clear path from assessment and strategy through implementation and optimization.
Each engagement builds on the ALIGN™ Framework and is tailored to the specific needs of your system.
Understand where patients are being lost—and why.
A focused engagement to evaluate your current navigation approach and identify gaps in access, coordination, and follow-up.

Breast Health ALIGN was founded by Dr. Patricia Flowers, a biomedical scientist, public health professional, and breast cancer survivor dedicated to improving access and equity in breast health.
Her work sits at the intersection of science, patient experience, and health equity—bringing a systems-level perspective to how patients access and move through care.
How Breast Health ALIGN Supports Health Systems
Breast Health ALIGN partners with hospitals and health systems to design community-integrated navigation models that extend beyond clinical settings and reflect the realities patients face in their everyday lives.
By connecting healthcare systems with community resources, ALIGN helps organizations strengthen access, improve equity outcomes, and ensure navigation reaches those most at risk of being lost along the care continuum.
Why Organizations Partner With Breast Health ALIGN:
PhD-trained biomedical scientist with clinical research experience
Public health training focused on health education and access
Experience supporting patient-centered initiatives within a national breast cancer organization
Breast cancer survivor with firsthand understanding of patient navigation challenges
Expertise in aligning healthcare systems, community resources, and patient needs
Grounded in both evidence and lived experience, Breast Health ALIGN designs practical, patient-centered strategies aligned with how care is accessed, coordinated, and experienced across settings.
Dr. Patricia Flowers brings a unique combination of lived experience, scientific training, and national-level experience in breast cancer advocacy to her work. With a PhD in Biomedical Sciences, training in public health, and experience supporting patient-centered initiatives within a national breast cancer organization, she has worked at the intersection of research, patient engagement, and health equity—helping shape how patient perspectives are meaningfully integrated into healthcare and industry. She is also a breast cancer survivor who brings firsthand insight into the realities patients navigate beyond clinical care.
Her experience includes contributing to community-based cancer research and care delivery within a national NCI-supported network, providing direct insight into how access, participation, and outcomes are shaped across diverse populations and care settings. This experience provides a systems-level understanding of how care delivery functions across settings, and where breakdowns in access and coordination can lead to gaps in patient follow-through.
Even well-resourced programs may not always have the capacity to address the full range of barriers patients experience outside of clinical settings. Her work centers on how everyday realities—such as transportation, language, insurance instability, trust, and coordination—continue to shape access, and why improving outcomes requires approaches that extend beyond traditional models. Through Breast Health ALIGN, she applies this expertise to strengthen how healthcare systems, community-based organizations, and industry engage with patients, ensuring that navigation and support are structured around real-world needs and experiences while meaningfully integrating the patient voice.
Her approach is grounded in the belief that effective solutions require both scientific rigor and practical application. By combining evidence-based strategies with lived experience, Dr. Flowers supports organizations in developing approaches that are not only well-designed, but also feasible to implement, responsive to community needs, and capable of driving measurable, sustained impact.
